- Doubling up after a missed dose. Unless the doctor says otherwise, taking two to "catch up" is one of the most common causes of accidental overdose we encounter.
- Two doctors, two prescriptions, one drug. A specialist and a primary doctor can each prescribe the same class of medication under different brand names. Nobody notices — until it becomes a problem.
- Mixing in over-the-counter meds and supplements. "Natural" doesn't mean harmless. Common supplements and OTC painkillers interact with blood thinners, blood pressure medications, and more.
- Using multiple pharmacies. Pharmacy software catches dangerous interactions — but only for prescriptions it knows about. One pharmacy = one safety net.
- Expired or discontinued medications kept "just in case." Old bottles get confused with current ones. Clear them out.
- No system. Loose bottles in a drawer means missed doses and double doses. A weekly pill organizer — filled the same day each week — solves most adherence problems.
- Nobody has the full list. In an emergency, the first thing responders ask is "what medications do they take?" A current, written medication list on the refrigerator can genuinely change outcomes.
